William and Kate Make Surprise Visit to a Hospital — But One Detail Has Royal Fans Worried

Royal watchers scrutinize every detail when the Princess of Wales steps out into public view — and this time, it was a missing accessory that prompted a familiar round of questions as she resumed her royal duties in 2026. On January 8, Prince William and Kate Middleton carried out a surprise visit to Charing Cross Hospital in west London, their first engagement of the year. As joint patrons of NHS Charities Together, the outing was meant to support the efforts of the hardworking staff of the NHS under extreme winter pressures. The visit was a working one, marking Middleton’s gradual return to the social scene.

Visually, the Princess struck a familiar note of quiet authority. She wore a fitted maroon suit with a burgundy blouse, all topped off and matched with accessories that felt practical and appropriate for a hospital. But to anyone who regularly follows what the royals wear, one detail jumped out almost immediately — her missing engagement ring.

Instead, the Princess wore only her Welsh gold wedding band. The sapphire engagement ring that once belonged to Princess Diana is arguably one of the most famous pieces of royal jewelry in the world, along with the white gold and diamond Annoushka ring she has worn prolifically over the past few years.

Nevertheless, the reasoning in this instance might be attributed to functionality. Operating environments require keeping ornaments to a minimum for infection control, and Middleton has consistently demonstrated that she is prepared to change her looks to accommodate the context. 

This is not the first occasion on which the Princess of Wales has left her engagement ring in a hospital setting. In 2018 and 2020, on two other occasions, she simply wore her Welsh gold wedding ring and decided not to wear any other jewelry. Another moment came in August 2024, when the Prince and Princess of Wales appeared in a light-hearted video celebrating the Olympic Games alongside Snoop Dogg. Eagle-eyed viewers quickly noticed that Middleton was not wearing her famous engagement ring — the 12-carat Ceylon sapphire encircled by 14 diamonds.

Instead, she paired her Welsh gold wedding band with her white gold and diamond eternity ring, stacking them alongside a separate gemstone-encrusted band whose exact details were difficult to discern on screen. While the missing sapphire inevitably prompted speculation, it was far from unprecedented. Since publicly announcing her engagement to Prince William in December 2010, Kate has worn the ring consistently — but not without exceptions. Over the years, she has regularly removed it during hands-on or physically demanding engagements, from digging in the dirt at outdoor projects to taking part in more active pursuits.

The same pattern emerged during the pandemic, when Middleton frequently appeared in video messages without the engagement ring. In early 2020, during a visit to Evelina London Children’s Hospital to meet patients with immune conditions, she again wore only her Welsh gold wedding band. At the time, her spokesperson told HELLO! that the decision had been made for hygiene reasons.
