Middleton has been a patron of the Forward Trust since 2021; today she was introduced to the charity's new initiative, Forward Growing, which promoted gardening sessions to connect people with nature. She emphasized the importance of discussing addiction openly to reduce stigma and support those affected, promoting kindness and understanding through the Taking Action on Addiction campaign. "By talking about it in the open, together we can bring addiction and the harm it causes out of the shadows," she stated.
